Humeralfracture

1

This patient has suffered a fracture of both upper arms as a result of a fall. The patient (very old) is in a moderate condition and therefore it was decided to perform the smallest possible operation. An intramedullary pin is then a good possibility.

The pin consists of metal and is introduced in the medullary canal as a result of which the fracture is stabilised. To prevent the bone from twisting around the pin,  bolts are introduced through the pin. Because the fracture of the left upperarm is quite distal, aftertreatment with a cast is necessary after the operation.

At the operation the arm is covered sterile (2). Here it concerns the right arm. By means of a small incision at the shoulder the skin is opened and the muscle is cleaved and the bone is reached(3). Then  access to the marrow cavity is obtained with a punch (4).

Hereafter a guide wire is introduced passing the fracture until the elbow is reached. The length of the pin to introduce is measured (5). The pin is fixed on a special introductiondevice (6) and is introduced. Afterwards the bolt screw is inserted(7).

After the operation only a pair of small wounds remain. The lower wound is of the introduced boltscrew.
