Conventional right hemicolectomy

If a tumor is situated in the right part of the colon, it is necessary te remove this part, the socalled right hemicolectomy (red in 1). In 2 the situation during the operation is shown.

If the tumor is malignant it is obligatory to also remove the tissue that contains the blood- and lymfvessels running to and from the colon (3,4). In 5 and 6 the small bowel is cut with a special device that staples and cuts.

Then the right colon is detached from the abdominal wall and tilted toward the left side of the patient. The duodenum and the liver with the gallbladder can then be seen (7). As a last step the colon is cut with the device (8,9).

The healthy parts of the gut are then hold together with a couple of sutures (10). The same device is then used to make a connection between the two parts, the anastomosis. The remaining defect is also closed with the device (13). Suturing with needle and thread is also possible, but the device is faster.

Right hemicolectomy is also possible with laparoscopic surgery. Once the colon has been freed intra-abdominally it is taken out through a small incision and resected. Then the anastomosis is made in the same manner as in the conventional way here above.
